Men's Basketball walk-on tryouts scheduled for Sept. 25 at SIU Arena

CARBONDALE, Ill. - The Southern Illinois men's basketball team will hold walk-on tryouts on Monday, Sept. 25, at 6:30 p.m. at SIU Arena.
To participate in these try-outs, a prospective student-athlete must complete the following forms, in advance. There will be no makeup date. All four forms are available on the website SIUSalukis.com.
*Try out physical. This form must be completed by a physician or physician assistant within the 6 months prior to your tryout. If you choose to get your physical exams using other forms, you must still use the history portion of the SIU's physical form. You must have a copy of this form with you at the tryout or turned in to the coach prior to the tryout.
*Try out Waiver. Complete this form including the dates of the try-out. You must have a copy of this form with you at the tryout or turned into the coach prior to the tryout.
*Student-Athlete Family Insurance Form. Complete this form and bring it and a copy of any primary insurance to the try out. You must have proof of insurance to participate in the try out. Student-athletes on the roster must carry the SIU Extended Medical Benefit.
*Application to practice. Follow the steps on the form in order to be approved to tryout.
